The City of Grande Prairie invites community members to attend upcoming engagement sessions to share their thoughts and feedback on a proposed Coordinated Care Campus to be located at 12102 100 Street.

The Coordinated Care Campus is a proposed partnership between the City of Grande Prairie, other levels of government, and community service providers.  The project involves the repurposing of a hotel and conference centre, located at 12102 – 100 Street into a Coordinated Care Campus to provide office and programming space for social service organizations, on-site health care and community supportive housing units.

The vast size of both the building and land provide a unique opportunity for the Coordinated Care Campus to conveniently co-locate residents with service providers, food, recreation, programming, designated Indigenous cultural space and healthcare all within one site.

The Coordinated Care Campus model offers onsite client care with wrap around 24/7 supports to address varied levels of need related to mental health, physical health, and addiction. The program includes group programming, life skills training, and assistance in accessing additional services. Additionally, the site is designed to be conducive to social enterprise and employment opportunities for program participants. All programming components have a focus on a recovery-oriented system of care.
